Well to be honest I did not score well in my degree but failure teaches you a lot. In Physical Chemistry you will find derivations which include very complex Differentiation and integration formula written one after another by skipping steps which may feel very unintuitive unless you are really good at mathematics. Just practice it multiple times so that you do not forget the steps (do not go too deep in deriving the formula itself, you will get frustrated if you do not like mathematics). So, inorganic and physical Chemistry papers normally woupd not hurt you much if you keep yourself updated daily by revising what you have done for the day. The real effort that you may require to put is in organic Chemistry because of the variety of questions that can be created using the chemical reactions. Thus when you are taught any topic in organic Chemistry you better read it again from a standard book and try to solve chemical equations based on the topic. You also need to keep in mind that questions can be framed by combining different chemical reactions. So try to create interconnections between topics which contain chemical reactions.

Direct admission to Engineering 2nd year is possible through Lateral Entry, for Diploma in Engineering students. Students who have completed 3 3-year Diploma course can take direct admission in engineering the second year. The admission is done through a process called the Lateral Entry Scheme. The eligibility condition for the same is as follows: The candidates seeking admission through this scheme should have completed the State Diploma Course of 3 years and should have secured an aggregate of 60% marks aggregate (50% for SC/ST and 55% for OBC). Depending on the aggregate percentage you have secured in your Diploma course, you will be shortlisted and then called for counseling. Also, if admitted you will directly start off from the second year of Engineering.
