The total number of questions in CAT exam is 100. So for 95% you will have to give 95 correct answers and not a single wrong answer.

For CAT, it is important to score more than 95 percentile. For MAT/CMAT, a score above 80 percentile is enough for you get good B-school.

Yes, most of the MBA offering institutes requires entrance exam scores. It may be CAT or any other. IIFT conducts IIFT entrance exam, similarly TISSNET for TISS institutes. Symbiosis conducts its own exam SNAP XAT for Xaviers and NMAT for Narsee Monjee institutes. It's better to write entrance exam to get a chance for a seat in top B-School of India.

Getting at least 95+ percentile should fetch you call from some IIMS (new ones). To convert the final call, scoring 98+ would help you a long way.

The scores of the HSC and SSC are considered for IIM-Bangalore and few others. But IIM-Ahmedabad, Calcutta, etc., give the admission on the basis of the CAT score.
