However, the application process for all the courses varies from each other. Application process for BSc & BPT:Step 1: Visit the official website of the institute and download the application form along with the information brochure.Step 2: Fill in the application form with all the required details.Step 3: Submit the application form online.After the online submission, take a printout of the filled-in application form and send it to 'Nodal Officer, College of Technology & Allied Health Sciences, SGPGIMS, Rae Bareli Road, Lucknow 226014' along with all the relevant documentsApplication Process for MD, MSc & MHA:Step 1: Visit the official website of the institute and click on the ‘Online Application form’ tab on the Homepage.Step 2: Fill it with all the valid information.Step 3: Send the application form to the correspondence address along with the application fee in form of a DD, along with other required documents. The envelope should be super scribed with the advertisement number and ‘Application for Entrance Exam to MHA Programme’. Application Process for DM, MCh & PDC:Step 1: Visit the official website of the college and click on the 'Online Form Submission' tab on the Homepage.Step 2: In the next window that opens, enter all the required details and click on the 'Register and Pay Online' button.Step 3: Pay the application fee online.Step 4: After one day of the fee submission, log on to the official website of the college.Step 5: Enter your transaction number and date of birth to open the application form
