IIMs consider not only CAT score but also your graduation, class 10th and class 12th score too and it has different weightage.

Your class 12th score are below par for getting a call from IIM A/B. Other IIMs might call you in case you manage to secure a very high percentile.

It has a huge importance when it comes to admissions into top IIMs. For instance, it constitutes 40% of the final score for deciding the admission for a student at IIM-A.

Yes, you can, provided you clear the cutoff and IIM A considers you good enough to give you a call! Being a doctor, you might get a call at low percentile compared to other engineering graduates! We do have a few doctors in every PGP batch.

Your acads are not upto the mark for IIM A. Having said that you can always apply for colleges which do not give high weight-age to acads like FMS.

First of all, none of the institute can offer you integrated MBA for the duration of 4 years. After AICTE directive, the minimum duration of any integrated MBA program is minimum 5 years whether you do a BBA+MBA or B.Tech+ MBA
Following are the options:
Nirma University (tier 3 institute)
NMIMS Mumbai offered in family business (no placement as this meant for those who already have family run business).
MBA Tech in IT Electronics and telecom, Chemical, Computer engineering, manufacturing and mechanical engineering, Civil engineering (5 years), open only for those with 10+2 with physics chemistry and mathematics.
B.Pharma + MBA pharma tech
...more

To answer your second question, of course it is possible. You'll have to first take CAT.
