Final selections is based on a cumulative score of your written exam, Interview/GD and profile. If you score well in the other two areas with a weak academic score, you may still get in.

Prepare as you would prepare for CAT. Also go through newspapers daily to prepare yourself for the General Knowledge section. Take mock tests.
Institution question is a bit vague. All IIM's are good along with IIFT, MDI, FMS, NITIE, NMIMS, SIBM, XLRI or any other institute that regularly appears in the top 20 list or top 30 list of MBA colleges
