AIIMS Mangalagiri offers 125 MBBS seats for Indian Nationals annually, with 53 allocated for the General Category. The remaining seats are reserved for Scheduled Castes, Scheduled Tribes, Other Backward Classes, and Economically Weaker Sections, following government regulations. There are 18 seats reserved for Scheduled Castes, 9 for Scheduled Tribes, 34 for Other Backward Classes, and 13 for Economically Weaker Section. A 5% horizontal reservation may be made for Person with Benchmark Disability under the Rights of Persons with Disabilities Act, 2016. Seat allocation may change slightly annually. High competition for these seats is expected, and securing a seat requires good performance in entrance exams like NEET.

For the academic year 2023–2024, the Kaloji Narayana Rao University of Health Sciences (KNRUHS) has made available the seat distribution for MBBS admissions under the Competent Authority Quota. There are 5513 seats available in total for MBBS admissions under this quota. Of these, 330 seats are designated for MBBS Minority Colleges, 203 seats for MBBS EWS Quota, and 4980 seats for MBBS (PWD, CAP, PMC, and SCCL) Quota. This year's MBBS admission seat distribution is divided into several categories, according to KNRUHS, including MBBS PWD, CAP, PMC, SCCL, EWS Quota, and Minority Colleges. The MBBS (PWD, CAP, PMC, and SCCL) quota receives the most seats, followed by the EWS and minority institutions.
