To be honest its unpredictable. But yes I think you can get colleges in Telangana easily. This year cutoff is also low.

IPU MBBS counselling is conducted in an offline mode. However, last year due to covid 19, the counselling was conducted in an online mode. I must apologise! But hold onto your optimism. The likelihood of obtaining a medical seat in a government medical college with a score of 237 is quite low. However, there is still a potential that you will get admitted to a private medical school. For a seat in the State Quota, you must score at least 450; for a place in the All India Quota, you must score at least 500. It is a little harder with 230. Since you have not provided your category, it is difficult to accurately answer whether you will get admission to AIIMS or not. The previous year's NEET UG cutoff score for AIIMS Madurai, is as follows:
1. General category: 645
2. OBC category: 640
3. EWS category: 634
4. SC category cut off: 554
5. ST category cut off: 524
As you can SEE from this data, you can get admission to AIIMS Madurai if you belong to the SC or ST quota. Also, generally, to get admission to AIIMS one needs to score higher than 600.

Yes, you can get admission to medical colleges in Telangana with a 446 NEET score. However, you have better chances of getting admission if you have the Telangana State Quota with your rank. You may get admission to the following college:
You can get a government seat at:
1. Ayaan Institute of Medical Sciences for the MBBS course
You can get a seat in the following dental colleges for a BDS course:
2. Mamata Dental College
3. MNR Dental College
If you can afford it, then you may also get a private seat at select colleges. For instance, you can get admission to the Apollo Institute of Medical Sciences and Research according to previous year's

Since you are from Telangana state, you will be treated as a General category candidate in NEET PG counseling in Karnataka state, irrespective of your OBC category. According to the latest eligibility norms, you need to have Karnataka as your domicile state to be qualified for the home state quota where category-based relaxations are provided. You may apply under the HKR quota if you belong to certain districts of the Hyderabad-Karnataka region, where an 8% quota is provided if you have a HKR Certificate. Otherwise, you must apply as an outside candidate for Karnataka NEET PG counseling in the open category.

So, students who are expecting more than 635 marks can expect to get a seat in a top medical college. It can be very difficult for students who get less tha 400 marks to get admission in a government medical college. However, a seat in dental colleges may be an option for them.
