You can definitely get admission into Kerala's government medical college for MBBS with a rank under 45. A 45 rank in NEET UG is a very decent rank, which can make you eligible for admission to top medical colleges within the All India Quota. Yes, you may have some chances to get admission into the Assam Medical College (AMC), Dibrugarh. This estimate is based on previous-year NEET UG cutoffs at Assam Medical College. The NEET UG cutoff for Assam ST (P) category in 2021 was 412 marks for MBBS. Also, the closing rank was 171906; according to this, there is some Probability that you may get admission to Assam Medical College. However, this estimate is based on the previous year, and this year's cutoff may vary, which will be soon announced.

With a 9576 NEET rank, you may not get admission into AIIMS Mangalagiri. AIIMS Mangalagiri generally has a higher admission cutoff and in the final round of 2021 had a closing rank of around 6544 for the OBC category. However, you may get admission into some other top government medical colleges for MBBS like Koppal Institute of Medical Sciences. With a score of 285, it is less likely that you will get admission to any government Ayurvedic college via the All India Quota. This is because to get admission in the Bachelor of Ayurvedic Medicine and Surgery (B.A. M. S. ) in the All India Quota, one has to score more than 350 in the NEET UG. You do have few chances to get government Ayurvedic college in state quota. For instance, you may get admission to Swami Kalyandev Government Ayurved College in Uttar Pradesh if you have a UP State home quota. Alternatively, you may apply to private colleges like Dr. Rajendra Gode Ayurved College and Hospital in Maharashtra.

You have plenty of options available with your NEET rank of 651897 to get admission into an MBBS college. According to the information given by you, you can get admission into the following colleges in Maharashtra for MBBS:
