Following is general information about Ph.D. admission, with some variation between different universities. Please check for your eligibility to pursue Ph.D. from the criteria mentioned below:
You should be a graduate with minimum 50% marks
The university advertises about research openings in specific areas and you can apply for a specific field, if your qualifications match. (If you know some of the faculty in the specific field, you are better informed about supervisors, openings etc. )

Firstly, you need to decide where you wish to apply. Thereafter, see if you meet their criteria for pursuing Ph.D. The minimum qualification required is PG or some national level exam such as GATE JRF etc. You must have obtained either a minimum of 50% marks or equivalent grading in the M.Phil. Degree or a minimum of 55% marks or equivalent grading in the Master's degree.

Now, there is one more course called direct Ph.D which most of the students didn't know about. Direct Ph.D is the course which you can directly do after B.Tech/be degree. The admission for direct Ph.D is same as MS admission and it is now there in all NITs/IITs. In the respective IITs, you can fill the form and they will call you for the interview and written test and based on your performance you will get the college. No, it is not mandatory to have GPAT/NET score. If you have done your Master's degree from M.Pharma or from M.Pharma with atleast 60% marks, then you are eligible for NIPER.
