A couple of well-known universities for physiotherapy include University of Connecticut, Arizonaa School of Health Sciences, FAMU, Alabama University, Georgia State University, etc.

To pursue Ph.D, you need to have minimum 60% marks in your post graduation.

IGNOU is one of the best universities to do your PhD in any given field.

The first and basic qualification required for a Ph.D. course is that
you must have completed your Masters degree course.
To apply for Ph.D. you can either directly apply to the professors of
different universities sending them your resume , Statement of purpose
and your research papers. Or you can appear for UGC-NET examination
and then sending your NET scores to the professors under whom you want
to undergo your Ph.D. course.

Yes, you do, however you need to demonstrate enough research work, interests, patent work, etc. to be accepted. Check with the DAAD office and try to attend the workshops, etc. before applying to any university. See here:
