You may check Wake Forest University, University of Massachusetts- Amherst, Tulane University, etc. It will be difficult to point out any university, different universities have different requirements. Most importantly universities in USA have a holistic approach that they will not only check your SAT score but every aspect of your profile from your academics to extracurricular activities. Your SOP and LORs will also be considered. You should start visiting university websites and check the requirements and shortlist those who have requirements close to your profile.

They don't release any forms. You must register and create an account on the official websites of SAT and TOEFL, pay the exam fee, select the test date and center, that's it. Check college board website and ets.org/TOEFL.

First & foremost it's great to know your dream of studying abroad. The admission process begins almost one year prior to the commencement of the course. US should be your destination. There are plenty of institutions. However, I would advise you to go to a specialist agency dealing with sending students to the US since there are many nuances to be observed.

They will consider you if you clear your SAT and TOEFL. If you are confused you can apply towards what is called and undeclared major in USA. You can apply to a Bachelor's degree but not declaring any specific concentration or specialization. You can later choose after you have explored the courses, subjects and also you will know your aptitude an interest. This will also depend on your test scores and academic performance. Yes, you can take the SAT and re-attempt it if you are not happy with the scores, so taking the SAT in 11th is actually a good idea.

For SAT and ACT: It depends on your proficiency and your target score. If you are aiming for the top 10 percentile and also are quite proficient already, then spend 3 hours for 2 weeks solving papers and revision. If you have just started preparing, then spend 2 hours for 3 months to gain good understanding of the test pattern and to do well.

If you want to study abroad then you need to specify or mention which country you want to study in. In USA SAT or ACT is compulsory while it may not be required for many other countries.
