Yes, you can apply. Most universities will ask for a GRE, you must also take the TOEFL. Some may even ask for atleast 1-year of work experience. Since every university has different admission requirements, you have to check their websites to have a better idea.

You have to clearly mention which program you want to apply to for a general overview, Bachelor's or Master's? Or you can check the respective program website on the university's website.

For that, you should contact the head office and ask about your related concern. There are many universities in America, which have tie ups with LPU, so it will not be a tough task for you to get admission in your desired college. Further, if your request from the head office will be sent to the international cell and then further formal proceedings will be there. Though, you are first advice to contact the head office or the international cell.

Yes, you can get admission in MBA in American universities. You need to work on developing your profile like taking GMAT and scoring well in that. You will also have to work hard on your resume, essays/SoPs and recommendations from your work and college. Fees for MBA may vary anywhere between 20,000 USD to 50,000 USD per year. Apart from this there will be about 12,000 $ living expenses. You can try for scholarships.
